Information Available: A systematic review of the effectiveness of selectedpopulation-based interventions addressing diabetes. The chosen programs fitwithin two strategic areas: health-care system level interventions and/ordiabetes self-management.

Description: The Diabetes Control Program, a public healthprogram in the California Department of Health Services, works to prevent anddetect diabetes, improve quality of care, develop and sustain public healthpolicy, reduce health disparities and raise diabetes awareness.

Information Available: Successful diabetes projects, facts and figures,publications-including a 2002 Basic Guidelines for Diabetes Care Packet(includes essential components of diabetes care, offers evidence-basedrecommendations), statistics and links to other diabetes resources.

Description: The NGA Center for Best Practices tracks stateefforts to prevent and manage diabetes. The Center provides research andanalysis for states on emerging issues and state best practices.

Information Available: Latest news concerning diabetes, publications from theNational Institutes of Health, general information, alternative therapies,diabetes journals and newsletters, clinical trials, symptoms, preventionprograms, research, statistics, law and policy, specific populations andreports on the management of diabetes.

Description: The National Institute of Diabetes andDigestive and Kidney Diseases conducts and supports research on many of themost serious diseases affecting public health. The Institute's Division ofIntramural Research encompasses the broad spectrum of metabolic diseases suchas diabetes, endocrine disorders, digestive diseases, nutrition, renal diseaseand hematology.

Description: Alliance to Reduce Disparities was launched byThe Merck Company Foundation — the philanthropic arm of Merck & Co., Inc toaddress the growing problem of health care disparities in the context of type 2diabetes in the United States among low–income and underserved adultpopulations with a commitment of $15 million through 2013.The Alliance aims tohelp decrease diabetes disparities and enhance the quality of health care byimproving prevention and management services.

Kansas City - Chronic Disease Coalition (KC-CDC)

Description: The KC-CDC is a community-based network thatexamines ways to promote health within the neigborhoods, churches and thecommunity of Kansas City, Missouri. The KC-CDC works to address the highprevalence of diabetes and cardiovascular disease among African Americans andHispanics. The organization hopes to create environmental conditions thatpromote widespread behavioral changes and improvement in population-leveloutcomes associated with the high prevalence of these diseases.

St. Louis Diabetes Coalition (SLDC)

Description: The SLDC was created to address the enormouschallenge of diabetes awareness and care through an integration of efforts andresources. Representatives of SLDC include: managed care organizations,physician groups, state and community health programs, the American DiabetesAssociation, academia, business and professional organizations, hospital groupsand pharmaceutical industry leaders.
