Description: The patient centered medical home (PCMH) is an approach to providing comprehensive primary care for children, youth and adults. The PCMH is a healthcare setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate the patient’s family.  The AAP, AAFP, ACP, and AOA, representing approximately 333,000 physicians, have developed the joint principles to describe the characteristics of the PCMH.

Description: The American College of Physicians (ACP) is a national organization of internists-physicians who specialize in the prevention, detection and treatment of illnesses in adults.  ACP is the largest medical-specialty organization and second largest physician group in the United States.  Its membership of 126,000 includes internists, internal medicine subspecialists and medical students, residents and fellows.

Description: The Patient Centered Medical Home model adopts components from the Chronic Care model.  The model rewards the prevention of illness and coordination of care. The medical home model is seen by many health-care providers, businesses and patients as a promising way to address problems with the country's health-care delivery system. It's believed that comprehensive primary care can ensure the best outcomes for patients.

Description: The Institute for Family Centered Care collaborated with the Institute for Healthcare Improvement to convene an expert panel on how to most effectively partner with patients and families in quality improvement and health care redesign.
